Kollo Balance+ and HRT: what you need to know before combining them
Many women considering Kollo Balance+ are already taking Hormone Replacement Therapy (HRT) or thinking about it. It's a reasonable question — can you take both, and is it a good idea? The honest answer is: it depends, and this is one of the situations where a conversation with your GP or pharmacist genuinely matters. This article walks through what's actually in Balance+, how each active ingredient sits alongside HRT, and what you should discuss with your healthcare provider before combining them.
Menopause and HRT — the current picture
Menopause is a natural life stage typically occurring between ages 45 and 55, marking the end of reproductive years. The transition (perimenopause) can span several years and involves declining oestrogen and progesterone production, which can cause hot flushes, night sweats, mood changes, sleep disturbance, vaginal dryness, joint aches, cognitive changes, and increased long-term risk of osteoporosis.
HRT (Hormone Replacement Therapy) is the NICE-recommended first-line treatment for menopausal symptoms in women for whom it's suitable. It replaces the hormones your ovaries have stopped producing (oestrogen, usually combined with progesterone if you still have your womb). NICE guidelines and menopause specialists broadly agree HRT is the most effective treatment for hot flushes and night sweats, and offers protective benefits for bone density.
Some women can't take HRT (for example, some with a history of oestrogen-sensitive breast cancer, unexplained vaginal bleeding, certain liver conditions, or specific clotting disorders). Others choose not to take it. For many women, the picture is somewhere in between: taking HRT and looking for additional support for symptoms not fully addressed.

Ingredient-by-ingredient: what to know if you're on HRT
Red Clover — the most important ingredient to discuss with your GP
Red Clover Extract
Discuss with GP if on HRTRed clover contains isoflavones — plant compounds that are structurally similar to oestrogen and can produce weak oestrogenic effects in the body. This makes it a phytoestrogen source, similar in category to soy isoflavones.
Why this matters with HRT: HRT is already providing your body with oestrogen (and often progesterone). Adding a phytoestrogen source on top could theoretically create additive effects — the concern is whether this changes what your prescribed HRT is designed to do. There isn't robust clinical evidence proving harm, but there also isn't clear evidence proving safety of combining the two. Menopause specialists commonly advise women on HRT to discuss red clover use with their GP before starting.
Additional considerations: Red clover may have mild blood-thinning effects. If you take anticoagulant medication (warfarin, DOACs like apixaban), aspirin, or other blood-thinning medications, please discuss this with your GP or pharmacist. Red clover is also not recommended if you have or have had oestrogen-sensitive cancers (like some breast cancers).
Regulatory note: EFSA has not authorised specific health claims for red clover in menopause. Traditional use for menopause symptoms is documented, but this differs from an authorised medical claim.
KSM-66® Ashwagandha
Ashwagandha Root Extract (KSM-66®)
Traditional adaptogenAshwagandha is an adaptogenic herb traditionally used in Ayurvedic medicine and now widely researched for stress adaptation, sleep quality, and general resilience. KSM-66® is a specific standardised extract with a substantial research base.
What to consider if you're on medications: Ashwagandha may affect thyroid function (potentially increasing thyroid hormone levels), so it's particularly important to discuss with your GP if you have a thyroid condition or take levothyroxine. It may also interact with sedatives, immunosuppressants, and blood pressure medications. No specific HRT interaction is widely reported, but full transparency about all supplements you take with your GP matters.
Not recommended: During pregnancy, or if you have autoimmune conditions like lupus, rheumatoid arthritis, or MS without medical clearance.
Siberian Ginseng (Eleuthero)
Siberian Ginseng Extract
Traditional adaptogenAn adaptogen traditionally used to support energy and resilience to stress. Not the same as Panax ginseng (Asian ginseng).
What to consider: Siberian ginseng may interact with blood-thinning medications (warfarin, aspirin), digoxin, and some diabetes medications. Not specifically flagged for HRT interaction but worth mentioning to your GP as part of a full supplement picture.
Chamomile
Chamomile Extract
Traditional calming herbChamomile is traditionally used to support relaxation and sleep. Generally very well tolerated at supplement doses.
What to consider: Chamomile may have mild blood-thinning effects — worth flagging if you take anticoagulants. Rare allergic reactions can occur in people with severe ragweed or daisy family allergies.
Sage Leaf
Sage Leaf Extract
Traditional use for hot flushesSage has some clinical research on hot flushes and night sweats, with a traditional use history for menopause symptoms. Generally well tolerated at supplement doses.
What to consider: Sage may interact with medications for diabetes or seizures — discuss with your GP if relevant. The dose in Balance+ (100mg extract) is well within traditional use ranges.
Lion's Mane, Dandelion Root, Hyaluronic Acid
Additional actives
Generally well toleratedLion's Mane (1,500mg) — mushroom extract traditionally used for cognitive support. Very well tolerated. Rare allergic reactions possible.
Dandelion Root (100mg) — mild diuretic effect at higher doses. May theoretically interact with lithium, diuretics, and some diabetes medications. Not a specific HRT concern.
Hyaluronic Acid (120mg) — naturally occurring molecule involved in tissue hydration. No known interactions with HRT or common medications.
Vitamins and minerals in Balance+
Vitamin B6, B9 (Folate), D3, and Magnesium
Standard nutritional dosesAll at nutritional (not therapeutic) doses. Under EU-authorised health claims (which still apply in the UK):
- Magnesium — contributes to normal muscle function, normal function of the nervous system, reduction of tiredness and fatigue, normal psychological function, and maintenance of normal bones
- Vitamin B6 — contributes to regulation of hormonal activity, reduction of tiredness and fatigue, normal function of the nervous system, and normal psychological function
- Vitamin B9 (Folate) — contributes to normal blood formation, normal homocysteine metabolism, normal psychological function, and reduction of tiredness and fatigue
- Vitamin D3 — contributes to maintenance of normal bones and teeth, normal muscle function, and normal function of the immune system
These are generally safe alongside HRT at the doses provided. High-dose vitamin D supplementation from multiple sources at once can accumulate — worth checking your total intake if you take other vitamin D supplements.
What to discuss with your GP or pharmacist
If you're considering Balance+ alongside HRT, useful things to raise with your GP or pharmacist:
- The full ingredient list of Balance+ (particularly red clover, ashwagandha, and Siberian ginseng)
- What type and dose of HRT you're on (oral vs transdermal, oestrogen-only vs combined)
- Any other medications you take, especially blood thinners, thyroid medication, diabetes medication, or antidepressants
- Your medical history — particularly any history of oestrogen-sensitive cancers, clotting disorders, thyroid conditions, or autoimmune conditions
- Whether there's a symptom Balance+ might help with that HRT alone isn't fully addressing (or vice versa)
When Balance+ might not be right for you
Regardless of whether you're on HRT, Balance+ isn't suitable for everyone. Please speak to your GP before taking Balance+ if:
- You have a history of oestrogen-sensitive cancers (breast, endometrial, ovarian) — because of the red clover content
- You have a clotting disorder or take anticoagulant medication
- You take thyroid medication (ashwagandha considerations)
- You have an autoimmune condition
- You are pregnant or breastfeeding — Balance+ is not suitable
- You are under 18 — Balance+ is not suitable for children
- You take multiple prescription medications with potential interactions
Balance+ as part of a wider approach
For most menopause symptoms, the strongest evidence-based approach is: proper GP assessment, HRT if suitable and desired, and lifestyle foundations (regular exercise particularly weight-bearing for bone density, balanced diet, sleep hygiene, stress management, alcohol moderation, and stopping smoking if applicable). Food supplements like Balance+ may complement this — they're not a replacement for the fundamentals.

Frequently Asked Questions
Can I take Kollo Balance+ with HRT?
Many women do combine Balance+ with HRT, but please speak to your GP or pharmacist before doing so. Balance+ contains red clover, which is a source of plant phytoestrogens — this could theoretically interact with the oestrogen in HRT. Your GP has your full medical picture and can advise whether it's appropriate for your specific situation, HRT type, and dose.
Is Balance+ an alternative to HRT?
No. Balance+ is a food supplement, not a medicine. HRT is the NICE-recommended first-line treatment for menopause symptoms in women for whom it's suitable — it works by replacing declining hormones. Balance+ contains botanicals traditionally used during menopause plus supporting vitamins and minerals, but doesn't provide hormone replacement. For women who cannot or choose not to take HRT, Balance+ may be considered as one option to explore with your GP among various approaches.
What does the red clover in Balance+ do?
Red clover contains isoflavones — plant compounds structurally similar to oestrogen that can produce weak oestrogenic effects. It's traditionally used to support menopause symptoms. Because it has this oestrogen-like activity, it's the ingredient that most needs to be discussed with your GP if you're on HRT or have a history of oestrogen-sensitive cancers.
How long until I notice benefits from Balance+?
Per Kollo's product guidance, most women take Balance+ daily for at least 4 weeks before judging how they feel, and 8-12 weeks for a full picture of how it fits into their routine. The botanicals and nutrients build up with consistent use. Individual responses vary significantly.
Is Balance+ suitable if I've had breast cancer?
Please speak to your GP or oncology team before taking Balance+ if you have or have had breast cancer or any oestrogen-sensitive cancer. The red clover content is the specific concern here — phytoestrogens may not be recommended in your case. Your medical team knows your specific cancer type, hormone receptor status, and current treatment, and can give you personalised guidance.
Does Balance+ contain marine collagen?
No. Balance+ and Kollo's marine collagen products are separate formulas. Balance+ is a plant-based (vegan) menopause supplement with botanicals, vitamins and minerals. Kollo's marine collagen products contain marine collagen from fish. Some women take both — again, please discuss with your GP if you're on HRT or medications.
Is Balance+ suitable for vegans and vegetarians?
Yes. Balance+ is registered with The Vegan Society and is suitable for both vegans and vegetarians. It's also free from gluten, dairy, lactose, soy, and wheat, and contains no added sugars, artificial colours, pork derivatives, or alcohol.
Can I take Balance+ during perimenopause, not just menopause?
Yes — Balance+ is formulated for women through both perimenopause and menopause. Perimenopause is the transitional phase leading up to menopause, and can bring many of the same symptoms. If you're experiencing significant perimenopause symptoms, please also speak to your GP about your options — perimenopausal HRT is available and can be highly effective for symptom management, and lifestyle approaches matter throughout.
